What Is the Organic Acid Test?

The Organic Acid Test (OAT) by Mosaic Diagnostics (formerly Great Plains Laboratory) is a comprehensive urine test that measures 76 organic acid metabolites — small molecules produced as byproducts of cellular metabolism. These markers provide a functional snapshot of how your body is actually performing at a biochemical level, revealing imbalances in energy production, neurotransmitter turnover, nutrient cofactors, detoxification pathways, and microbial overgrowth.

The OAT is one of the most clinically powerful functional tests available because it connects multiple body systems in a single panel — linking gut health, mitochondrial function, brain chemistry, and nutritional status in ways that standard blood work cannot.

What Does It Measure?

Krebs Cycle & Mitochondrial Markers
Including citric acid, succinic acid, fumaric acid, malic acid, and hydroxymethylglutaric acid. These reveal how efficiently your mitochondria are producing cellular energy (ATP). Abnormalities here are commonly seen in chronic fatigue, fibromyalgia, and post-viral syndromes.

Neurotransmitter Metabolites
Including homovanillic acid (dopamine), vanilmandelic acid (noradrenaline/adrenaline), 5-hydroxyindoleacetic acid (serotonin), and quinolinic acid (neuroinflammation). These markers reflect neurotransmitter production and breakdown — critical for patients with mood disorders, anxiety, ADHD, and cognitive complaints.

Yeast & Fungal Markers
Including arabinose, tartaric acid, citramalic acid, and others. Elevated yeast metabolites are among the most common findings on the OAT and are associated with brain fog, sugar cravings, bloating, and immune dysfunction. These markers often reveal Candida and other fungal overgrowth that stool testing misses.

Bacterial Markers (Clostridia & Others)
Including HPHPA, 4-cresol, and 3-indoleacetic acid. These indicate overgrowth of specific pathogenic bacterial species in the gut — particularly Clostridia species, which produce neurotoxic metabolites linked to mood, behaviour, and neurological symptoms.

Oxalate Markers
Including glyceric acid, glycolic acid, and oxalic acid. Elevated oxalates can indicate oxalate overproduction or poor clearance, which is associated with kidney stones, joint pain, and chronic inflammation.

Nutritional Cofactor Markers
Including methylmalonic acid (vitamin B12), formiminoglutamic acid (folate), xanthurenate and kynurenate (vitamin B6), pyroglutamic acid (glutathione), and ascorbic acid (vitamin C). These functional markers reveal whether your body has adequate levels of critical nutrients — even when serum levels appear normal.

Who Should Consider This Test?

This test is particularly valuable for patients experiencing:

• Chronic fatigue, low energy, or post-viral exhaustion
• Brain fog, poor concentration, or memory issues
• Mood disorders — depression, anxiety, irritability — especially if treatment-resistant
• Bloating, sugar cravings, or suspected yeast/Candida overgrowth
• Behavioural or developmental concerns in children (ADHD, autism spectrum)
• Fibromyalgia or chronic pain syndromes
• Nutrient deficiency symptoms despite supplementation
• Complex chronic illness where multiple body systems appear affected
• Anyone wanting the single most comprehensive metabolic overview available

Sample Collection

Sample type: First morning urine
Collection method: Collect first morning urine into the provided specimen cup; freeze immediately until ready to ship
Fasting required: No food or drink (other than water) from midnight before collection
Diet restrictions: Avoid apples, grapes, pears, and their juices for 48 hours before collection (these contain arabinose and can affect yeast marker interpretation)
Supplements: Discuss with Rohan — certain supplements (especially high-dose B vitamins and NAC) may need to be paused for accurate baseline results

Turnaround Time

Results are typically returned within 4 weeks from the date the laboratory receives your sample. Samples are processed in the USA by Mosaic Diagnostics.

What Happens After You Order

1. Kit dispatched — Your OAT collection kit is posted directly to your door with full instructions.
2. Collect at home — Follow the first morning urine collection protocol and freeze the sample.
3. Ship to lab — Ship the frozen sample to the lab using the provided shipping materials.
4. Results released to you — If you’re an existing patient, Rohan will go through the result with you at your next appointment. If you have your own practitioner, you’re welcome to order through Elemental Health and share the results with them directly.

Why Choose Elemental Health?

The OAT is one of Rohan’s most-used and most-valued functional tests — he considers it the single best metabolic overview available for complex chronic patients. Every result is reviewed in the context of your full clinical picture, symptom history, and other test results. Your consultation will include a clear, prioritised treatment strategy that addresses the highest-impact findings first, covering antimicrobial protocols, mitochondrial support, targeted supplementation, dietary adjustments, and any further testing needed.
